For Employees

Employees must notify their immediate management supervisor immediately if Employee has been diagnosed with COVID-19 virus and complete and submit the Employee COVID-19 Questionnaire located on myGateway..

For Students

Students must notify their Instructor if they have been diagnosed with COVID-19 virus. They must complete and submit the Student COVID-19 Questionnaire located on myGateway.
